BBC Trust chairman Michael Lyons lays the heaviest blame for the Sachs affair on Radio 2's senior editorial management.
No further action will be taken against Jonathan Ross following the broadcast of obscene phone calls he made with comedian Russell Brand to the actor Andrew Sachs.
Lyons told a press conference this morning that management had shown a serious lack of judgement.
Sir Michael also set forward proposals to prevent offensive material being broadcast in the future, including making a list of high risk programmes and imposing stricter penalties for breaching editorial guidelines.
The controller of Radio 2, Lesley Douglas, has already resigned over the scandal.
